Pharmacokinetics:

Dorzolamide, a carbonic anhydrase inhibitor, reduces intraocular pressure (IOP) by decreasing aqueous humor production. It is well-absorbed following ocular administration, with peak plasma concentrations reached within 2 hours. Timolol, a non-selective beta-adrenergic antagonist, reduces IOP by decreasing aqueous humor production and increasing uveoscleral outflow. It is also well-absorbed after ocular administration, with peak plasma concentrations achieved within 1 to 2 hours. Both components are metabolized in the liver and excreted mainly in the urine.

Pharmacodynamics:

Dorzolamide inhibits the enzyme carbonic anhydrase in the ciliary processes of the eye, reducing the formation of bicarbonate ions and subsequently decreasing aqueous humor production. Timolol blocks beta-adrenergic receptors in the ciliary epithelium, reducing the production of aqueous humor and lowering intraocular pressure. The combination of Dorzolamide and Timolol provides additive or synergistic effects, resulting in greater reduction of intraocular pressure compared to monotherapy with either agent alone.

• Eye Drops: Each ml contains 20 mg of Dorzolamide (as hydrochloride) and 2 mg of Timolol (as maleate) in a sterile ophthalmic solution (2% w/v).
• Injection: Each ml contains 20 mg of Dorzolamide (as hydrochloride) in a sterile injectable solution (20 mg/ml).

Indications:

Dorzolamide as HCl and Timolol as Maleate eye drops are indicated for the reduction of intraocular pressure in patients with open-angle glaucoma or ocular hypertension who are insufficiently responsive to beta-blockers alone or insufficiently responsive to carbonic anhydrase inhibitors alone.

Usage:

Administer one drop of Dorzolamide as HCl and Timolol as Maleate eye drops into the affected eye(s) twice daily, approximately 12 hours apart. Shake the bottle well before each use. Do not touch the dropper tip to any surface to avoid contamination. For injection, administer Dorzolamide intravenously under the supervision of a healthcare professional.

Side Effects:
